3. HACC Innovative Solutions Manual Innovative Solutions

In 2004, the NSW HACC DOs Network produced the first HACC Innovative Solutions Manual to showcase proven ideas for addressing common community care sector issues across NSW. This 2010 edition aims to ensure that the lessons learnt from creatively addressing common sector issues and problems in one area continue to be passed on to other areas.

AGED CARE PLAN: LIVING LONGER, LIVING BETTER Policy & Guidelines

On 20 April, the Australian Government released its 10 year plan to reshape aged care - LIVING LONGER, LIVING BETTER. Commencing on 1 July 2012, it includes establishment of an Aged Care Implementation Council, followed by an Aged Care Financing Authority and MyAged Care Website and call centre in 2013. The full Gateway to Aged Care encompasses assessment and linking services in 2014 and the Aged Care Quality Agency to produced ratings on residential aged care and the new Home Care Packages. HACC will join CACPs, EACH, EACH D, NRCP and ACHA under the new umbrella from 1 July 2015. For the full report and fact sheets, go to

Commonwealth HACC Program Manual Policy & Guidelines

This Program Manual sets out the operational requirements of the Commonwealth HACC Program from
1 July 2012. It is intended for all Commonwealth HACC service providers, although other interested groups
may find it useful.
This manual does not apply to HACC service providers in Western Australia or Victoria. The existing HACC
arrangements, including Australian Government funding, will remain in place for these states from 1 July 2012.
The manual outlines requirements regarding:
• the delivery of Commonwealth HACC services, including the aims of the Commonwealth HACC Program,
target groups and service types (Part 1); and
• the administration of the Commonwealth HACC Program (Part 2), including payment of grants, financial
reporting, insurance and so on.
Service providers delivering services from 1 July 2012 under the Commonwealth HACC Program must comply
with both the terms and conditions contained in their Aged Care Funding Agreement and this Program Manual.
The funding agreement will take precedence if there is any discrepancy with this Program Manual. (Chapter 1, pg 1, Commonwealth HACC Program Manual 2012)

HACC Program Position Statement on Palliative Care Policy & Guidelines

There have been concerns raised about whether palliative care patients are eligible for HACC services. This is because pursuant to clause 4(2) of the HACC Review Agreement certain services including specialist palliative care services are listed as outside the scope of the HACC program because funding is provided to such services through other government programs.

Ageing Disability and Home Care (ADHC) and NSW Health have jointly developed a HACC Program Position Statement to provide clarification to the eligibility of palliative care patients for HACC services.

IMPACT Working Group Communique Other

Two years after the first NSW Industry Group forum in July 2008, the IMPACTS Working Group is still going strong promoting and enabling a person-centred approach in the Home and Community Care (HACC) Program in NSW. We thought it was timely to give our community partners - worldly and wise HACC workers, and newcomers alike - an update on our activities, as we participate in the Better Practice Project of Ageing, Disability and Home Care (ADHC) in the NSW Department of Human Services....

New Guidelines for NSW Community Care Supports Program. Policy & Guidelines

From 1 July 2012, NSW has sole responsibility for community care services for younger people previously provided under the Commonwealth / State Home and Community Care (HACC) program. ‘Younger people’ are people under the age of 65, or 50 years for Aboriginal or Torres Straight Islander people.
The NSW Community Care Supports Program is funded and administered by Ageing, Disability and Home Care (ADHC). The CCSP provides basic community support services to approximately 54,000 people in NSW.
